The Gasteria pillansii is a "distichous" succulent, meaning its leaves grow in two opposite ranks, stacking neatly on top of one another like a fan—though older specimens may eventually form a rosette. The leaves are thick, fleshy, and tongue-shaped (hence the common name), with a distinctive rough, "sandpapery" texture.
The foliage is dark green, mottled with small, slightly raised white spots that form irregular transverse bands. In late spring to summer, it sends up a tall, branched inflorescence bearing pendulous, stomach-shaped flowers that are pinkish-red with green tips.
